Online Data Access Process
Data requests should be submitted to the Data Access Committee using only the online Data Request Form. The link to the online form will be provided following the initial discussion with the Project Management Group (PMG)
The online form is a smart form. Depending on the type of project you are planning, a sub-set of questions will open up. With the guides below you can use these offline word versions to complete your application and this can be shared during the discussion with the PMG and then copied to the on-line form. Do not submit the word version of the form for review by the DAC.
- To assist with designing a new project
- To obtain deidentified existing patient data from the registry
- Any other project
Data Request Justification Form
Question 28 requires you to specify and justify the data elements requested. Please complete the relevant section of the Data Request Justification Form which can then be uploaded to the online form.
Surveys
For projects that solely require a survey of participating centres and do not request extraction or transfer of patient-level registry data, applicants should contact the PMG at info@sdmregistries.org
