Participant information sheets (PIS) and informed consent forms (ICF) in several languages that are used at a range of centres and are based on the UK ethics approved documents are provided below as samples. These are revised every 5 years as a minimum and it is recommended that all local PIS and ICF are also reviewed every 5 years.
