If you require access to your record in SDMregistries, you can either Request a printout of your record directly from your hospital doctor or you can access the Registry yourself.
To access the registry, you need to provide your hospital doctor with an e-mail address. This can also be done through the SDMregistries Consent Form.
Once your email address has been added, your doctor will send you an electronic invitation through the Registry. You will then receive an email with a secure link to create your personal Registry account.
Once you have created your account, you can access it at any time via the log-in link on https://registry.sdmregistries.org/auth/login
